Pharmacy Technician Payment Plan Agreement – MSJC – September 2026

Terms of Agreement

1. Down Payment: I understand that a down payment of $899.00 is required in order to register for the program. (In the event of program cancellation, down payment will be refunded within 10 business days.)
2. Monthly Payments: I understand that I will make five (5) monthly payments of $360.00 and that payments are due as follows:
   a. Payment 1 – $360.00 due on October 5, 2026
   b. Payment 2 – $360.00 due on November 5, 2026
   c. Payment 3 – $360.00 due on December 5, 2026
   d. Payment 4 – $360.00 due on January 5, 2027
   e. Payment 5 – $360.00 due on February 5, 2028
3. Method of Payment: I understand that I will be invoiced each month for payments and payments may be made through the following:
   a. Mail – (Check or Money Order only)
   b. Square – (Credit Card)
4. Late Fees: I understand that a late fee of $25 will be assessed for any payment not received by the due date. Late fees are due at the time of payment and will be added on to the next payment if not paid.
5. Grounds for Dismissal: I understand that I will be dropped from the program if a payment is more than 30 days past due.
6. Collections: I understand that my account will be turned over to a collection agency for any payments that are not paid within 30 days.
7. Convenience Fees: A nominal convenience fee will be added to any payment through Square (1% for ACH and 2.9% for credit card).
8. Total Amount Due: I understand that the total amount due for the Pharmacy Technician program is $2,699 plus any assessed late fees and convenience fees, if applicable. I agree to pay the entire amount of $2,699 regardless of the outcome.
9. Refund Policy:  Career Training Solutions adheres to MSJC’s cancellation policy which is as follows: Refunds must be requested at least five business days prior to the program’s start date by emailing jflint@careertrainingsolutionsllc.com.  A $25 processing fee is charged for each program refunded. No transfers.  No makeup sessions for missed classes or late arrivals.  Registrations are non-transferable. Refunds are issued back to the original form of payment within 2-4 weeks after the request is received and processed. If we cancel a class, we will notify you, and a full refund will be issued by the original form of payment.
